2022 Physical Night:
We will be teaming up with Methodist Sports Medicine this year to offer FREE sports physicals to our student athletes, band members, cheerleaders, dancers and ROTC members.
Physical Night will be Wednesday, May 25th @ 4:00 pm.
In order for your student to receive a physical on May 25th, the following must be done
  • A signed Pre-Participation Exam release form turned in by May 20th
  • A completed Pre-Participation Medical History form turned in by May 20th
Only those students who turn in completed forms will be eligible for a physical on May 25th.
Walk-ups will not be accepted on physical night. Forms must be turned in ahead of time.
Forms can only be turned into the Athletic Department, the Athletic Trainer, coaches or sponsors. Do not turn in forms to the front office.
Forms can be picked up from the Athletic Training room or can be printed from the athletic website.

Injury documentation
In order to provide the best care possible for the student athlete, the Athletic Trainer must be kept abreast of all illnesses and injuries. Should your son or daughter go to the doctor please bring a note back with directives from the doctor about play guidelines. If a student athlete does not bring a note back, it will be assumed that the doctor said he/she was not allowed to play and the student will sit out until the note is received.
