Please bring any new immunization documation to your school nurse . If you have a student that is a sixth grader , they are required to have a Dtap vaccine that is less than five years old , a meningococcal vaccine ( MCV ) and two Varivax vaccines ( Chicken Pox ) or proof of chicken pox before the first day of seventh grade .
